You have not been cautioned, subject to a court order, bpound over, received a reprimand or warning or found guilty of committing any offence since the date of your most recent DBS or during your employment at this setting.
You are not disqualified for caring for children
You have not committed offences against a child, or an adult (eg rape, murder, indecent assault, actual bodily harm etc). You have not been barred from working with children.
You are not living with someone who had been barred from working with children or dis-qualified from working with children under the Childcare Act 2006.
Your own children have not been taken into care and your own children are not subject to a child protection order.
You have declared all medical conditions that could affect your ability to care for children.
